Registration Information Fall 2007 Meeting - St. Louis

How to Register  (two options)

MAIL:  Complete the Registration form and mail it with your check to:    (make your check payable to HAABB)

 WEB:  Register online at haabb.org.  Click on either Member Registration or Non-member Registration to use your credit card.  A 4.5% service charge will be added to your registration fee. 

Membership

If you are not an HAABB member, you can join HAABB and save $20 on your registration.  To join, complete the Membership form and mail it with your registration.

To join online, go to haabb.org.  Go to Membership Registration and choose Individual or Institutional membership. 

Upon receipt of your membership payment, you are entitled to all the benefits of membership through July 2008 and you may register at the member rate for the 2008 Annual Meeting in Kansas City.

You will receive a membership card as a receipt of your membership payment.

How to Complete the Registration Form

  1. If you are an HAABB individual member, please check the member box.
  2. If you are using your Institution’s membership, please check the Institution member box and fill in the Institution’s Name.
  3. If you are applying for membership, Individual or Institutional, please check the appropriate box and add that fee to your registration.
  4. Complete the Attendee information.
  5. Choose the sessions you will attend.
  6. Total your registration fees and membership fees and write the amount on Line 4.
  7. Make checks payable to HAABB. 

Institutional Members

  1. Institutional Membership entitles you to send multiple attendees using 1 Institutional Membership.
  2. You will pay the session fee for each attendee that you send.  Each attendee will be issued a name badge with the Institution Name.  When the attendee turns their badge in to Registration, another attendee may use the badge.

Please indicate the number of sessions you are paying for. 

Example:

         4  Wednesday AM

         5  Wednesday PM

Five Institutional Badges will be issued.  4 attendees can attend the Wednesday morning session.  5 attendees can attend the Wednesday afternoon session. 

Fees charged are:  $  30 Institutional Membership

                           $    450   for 9 half day sessions

                           $   480  Total

  1. All Institutional attendees are invited to attend lunch the day of their session.
  2. A jobs board will be available at the meeting for facilities to post any open positions.
